Arizona Foundation for Eye Health $30,000 Support for a second year of the Arizona Telemedicine Ophthalmic Program to screen and provide follow-up vision services for over 400 low-income patients.
* Banner Health Foundation $27,000 Support to expand the Banner Alzheimer's Institute-Native American Outreach Program to educate Arizona tribes, Urban Indian communities and other Native American organizations about the disease.
Beatitudes Center D.O.A.R., Inc. $50,000 Support for a second year for the Computer Assistance Program to provide computers, technical support, Internet connections and training to 250 homebound adults in the Valley.
Boys & Girls Clubs of Metropolitan Phoenix, Inc. $100,000 Support to sustain the after-school and evening food program that annually provides 300,000 snacks and 175,000 hot meals at all 12 Club branches serving low-income youth ages 6 to 18.
Child Abuse Prevention Fund $150,000 Support over two years to provide matching funds for revenue raised through the sale of the It Shouldn't Hurt to be a Child license plates, which will fund primary prevention programs in Maricopa County.
* Community Information & Referral $75,000 Support to maintain the 24-hour Help Hotline that receives over 23,000 calls each month from individuals and families in need of food, shelter, rent, utility and mortgage foreclosure assistance.
Eight, Arizona PBS $50,000 Support for a second year for Ready To Learn, a community-based early literacy program that provides free workshops, children's books and PBS magazines to over 990 parents and caregivers annually to prepare children for school in Maricopa County.
First United Methodist Church of Tempe $70,000 Support for a second year for the Urban Outreach Program to provide direct assistance to over 800 homeless youth and adults in Tempe.
Girl Scouts - Arizona Cactus-Pine Council, Inc. $50,000 Support for a second year for Adelante Jovencitas, a nontraditional Girl Scout program that annually serves over 600 juvenile female offenders and delinquent girls, ages 13 to 18, residing in juvenile correction facilities and group homes.
HomeBase Youth Services $100,000 Support to provide emergency shelter and transitional housing to 700 homeless youth, ages 18 to 21, and to teach them independent living skills.
Homeward Bound $75,000 Support for a second year for Project SAVE, which provides on-site counseling services to clients who are experiencing or at risk of substance abuse and relapse.
Interfaith Cooperative Ministries $25,000 Support to purchase a truck to transport food and clothing to nearly 55,000 individuals and working poor families in Central Phoenix.
National Wildlife Federation $49,000 Support for a second year for the Tribal Lands Conservation Program to implement schoolyard habitat projects at schools and community centers located on tribal lands throughout Arizona.
* Neighbors Who Care, Inc. $24,000 Support over two years for the mileage reimbursement program to recruit and retain volunteer drivers to provide transportation for over 370 homebound adults in the Sun Lakes community.
Newtown Community Development Corporation $30,000 Support for the Foreclosure Prevention Services program to provide foreclosure workshops and homeownership counseling.
* Paz de Cristo Community Center $45,000 Support to provide over 250 meals each evening and 1,300 emergency food boxes each month to homeless, near homeless and working poor individuals in the east Valley.
Phoenix Herpetological Society, Inc. $100,000 Support over two years to hire the sanctuary's first paid administrator to supervise protection for over 600 animals, oversee facility maintenance and improvements, and increase educational programs.
Sky Island Alliance $75,000 Support for Conserving the Sky Islands: Adapting to a Changing Climate, a collaborative project to protect the Sky Islands while creating models addressing the effects of climate change for future application throughout Arizona.
Sojourner Center $95,000 Support for the Save a Bed Campaign to maintain the current level of emergency shelter and support services for women and children who are victims of domestic violence.
Tumbleweed Center for Youth Development $85,000 Support to maintain programs for 3,000 homeless and runaway youth in Phoenix and Tempe.
UMOM New Day Centers, Inc. $100,000 Support for the Emergency Shelter Program that provides food, shelter, life skills and job readiness training to stabilize families in need.
